Avocare Health Information Network Florida Signup Process
We connect and serve local healthcare providers today while connecting to the state and national networks of tomorrow
To signup for Avocare's Health Information Network (HIN) service in Florida please complete the following three step process:
1. Register Organization 
Download and complete the Organization Registration Agreement (CLICK HERE TO DOWNLOAD). Additional information on Avocare HIN Services are also available for your review.
2. Authorize Users
Download and complete the User Application Form (CLICK HERE TO DOWNLOAD) for each user within your organization that will require access to the RHIN system. Once your organizaiton has completed the registration process there is no limit to the number of individual user accounts that can be provided.
3. Setup Training
Coordinate setup and training with Avocare staff by calling (850) 702-0094 and ask for Health Information Network services or emailing support@AvocareHealth.com.
